Physical Therapist Assistant (PTA)

We are currently seeking a qualified individual to join the Lucent Health, Home Health Services team. Someone who has the experience, knowledge, and leadership skills, who is dedicated and passionate about their career and the communities we serve, and who is looking for growth and advancement.


JOB RESPONSIBILITIES:

  • Provide skilled Physical Therapy services, on an intermittent basis, to patients, in their homes, under direction from the Registered Physical Therapist (PT).
  • Under the supervision of the PT, assist in evaluation and development of a rehabilitative plan of care and in periodic re-evaluation, as required.
  • Participate in the preparation of clinical and daily progress notes; all notes are reviewed by the PT and submitted, within 72 hours. Also prepares periodic summary reports.
  • As directed, perform routine strengthening, and therapeutic, exercises and assist the patient with the use of self-help devices.
  • Together with the PT, instruct the patient, and family, and assist in teaching exercise programs, to the patient and family.
  • Observe, and record, activities and findings, in the clinical record, and reports to the PT and the physician the patient's reaction to treatment and any changes in the patient's condition.
  • Consult with PT, regarding the outcome of home visits and report physical findings, in written form.
  • Attend rehabilitation department staff meetings and case conferences.
  • Participate with staff, patients, and physicians in discharge activities and assist the Physical Therapist in completing the Physical Therapy Discharge Summary.
  • Confirm, on a weekly basis, the scheduling of visits with the Supervisor/Clinical Manager in order to coordinate Agency visits.


JOB CONDITIONS:

  • Must have a driver’s license and be willing, and able, to drive to patients' residences.
  • The ability to access patients' homes, which may not be routinely wheelchair accessible, is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care.
  • Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.
  • The ability to communicate, both, verbally, and in writing as frequent communication, by telephone in English, is required.


QUALIFICATIONS:

  • Graduated from a Physical Therapist Assistant curriculum, approved by the Commission on Accreditation in Physical Therapy Education of  the American Physical Therapy Association
  • Passed a national examination for Physical Therapist Assistants.
  • Must be licensed by the State of Florida as a Physical Therapy Assistant.
  • Must have a criminal background check.
  • Must have a current CPR certification. Online certification is not accepted.


PAY:

  • Different pay scales based on service provided.
