What To Expect


Home health care is a system of care provided by skilled practitioners to patients in their homes under the direction of a physician. Home health care services include nursing care; physical, occupational, and speech-language therapy; and medical social services. The goals of home health care services are to help individuals to improve function and live with greater independence; to promote the patient’s optimal level of well-being; and to assist the patient to remain at home, avoiding hospitalization or admission to long-term care institutions.

During the initial visit, the clinician will complete a full assessment that covers comprehensive health areas which will help to build an effective plan of care to accomplish positive goals.



Some of the checks that our professional clinician will do during the assessments and scheduled visits are:

  • Monitoring the eating and drinking regimen you follow
  • Taking your vitals (Weight, Blood Pressure, heart rate, temperature, breathing)
  • Going through your medication
  • Asking if you have any pain
  • Going through safety measures at home

Clinician Visits

Depending on your needs, you will have different clinicians come to your home; these may include nurses, therapists, medical social workers, and home health aides.

Doctor Updates

Kindly note that we apply a very effective communication approach by keeping your doctor updated about your progress and changes in your health.

Admission Packet

During the first visit, our clinician will hand over to you a full admission packet that includes a copy of the Home Care Bill of Rights that outlines what your rights and responsibilities are regarding your care.
