Community Liaison
Job Overview: Responsible for interaction in the community and promoting company services.
Licensed Practical Nurse (LPN)
Job Overview: Provide skilled nursing care, under the direct supervision of the Registered Nurse.
Nutritionist (RD)
Job Overview: Direct, supervise, evaluate, and provide nutritional services, on an intermittent basis, to patients, in their homes, in accordance with the physician's orders.
Occupational Therapy Assistant (OTA)
Job Overview: Provide skilled occupational therapy service, on an intermittent basis, to patients, in their homes, under the supervision of the Occupational Therapist.
Psychiatric Registered Nurse (RN)
Job Overview: Responsible for furnishing comprehensive, skilled psychiatric nursing services to psychiatric patients in their place of residence.
Physical Therapist (PT)
Job Overview: Provide skilled Physical Therapy services, on an intermittent basis, to patients, in their homes, in accordance with physician orders.
Physical Therapist Assistant (PTA)
Job Overview: Provide skilled Physical Therapy services, on an intermittent basis, to patients, in their homes, under direction from the Registered Physical Therapist.
Registered Nurse (RN)/Case Manager
Job Overview: Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient.
Respiratory Therapist (RRT)
Job Overview: Perform assessment procedures to determine patient treatment needs, make recommendations for respiratory care and treatment, administer treatment procedures based on patients' needs.
Speech Pathologist (ST)
Job Overview: Provide skilled speech pathology service, on an intermittent basis, to patients, in their homes, in accordance with the physician's orders.
