Speech Pathologist (ST)

We are currently seeking a qualified individual to join the Lucent Health, Home Health Services team. Someone who has the experience and knowledge, who is dedicated and passionate about their career and the communities we serve, and who is looking for growth and advancement.


JOB RESPONSIBILITIES:

  • Provide skilled speech pathology service, on an intermittent basis, to patients, in their homes, in accordance with the physician's orders.
  • Establish a Speech Therapy treatment plan, in consultation with the physician, and revise it, with the approval of the physician, as necessary.
  • Assist in development, and implementation, of the interdisciplinary patient care plan, as it pertains to Speech Therapy.
  • Ensure that the physician's orders are appropriate and discuss necessary changes.
  • Teach, supervise, and counsel the family, and patient, in the total Speech Therapy program and other related problems of the patient, at home.
  • Record, and report to the physician, the patient's reaction to treatment and /or any changes in the patient's condition.
  • Evaluate the effectiveness of speech pathology intervention and updates, the patient care plan, as needed.
  • Participate in Agency activities, case conferences, and meetings, when appropriate.
  • Participate with staff, patients, and physicians, in discharge planning activities, and complete a Speech Therapy Discharge Summary.


JOB CONDITIONS:

  • Be willing, and able, to drive to patients residences.
  • The ability to access patients' homes, which may not be routinely wheelchair accessible, is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care.
  • Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.
  • The ability to communicate, both, verbally, and in writing as frequent communication, by telephone in English, is required.


QUALIFICATIONS:

  • Must be licensed in the state of Florida as a Speech-Language pathologist.
  • Preferably a member of the American Speech-Language and Hearing Association.
  • One, or more, years in Home Health Agency or in a hospital setting. Home health experience preferred.
  • Must have a criminal background check.
  • Must have current CPR certification. Online certification is not accepted
